Position Details


* Position: LVN Case Manager


* Schedule: Full-Time


* Annual Salary: $114,400


* Location: Medical Hill Healthcare Center

Key Responsibilities


* Coordinate and oversee resident care plans from admission through discharge.


* Collaborate with physicians, nursing staff, therapists, residents, and families to ensure continuity of care.


* Monitor clinical progress and identify opportunities to improve patient outcomes.


* Facilitate care conferences and interdisciplinary team meetings.


* Assist with discharge planning and transitions of care.


* Ensure compliance with federal, state, and facility regulations.


* Maintain accurate clinical documentation and case management records.


* Advocate for residents while promoting quality care and customer satisfaction.

Qualifications


* Current California LVN license in good standing.


* Case management, skilled nursing, or long-term care experience preferred.


* Strong assessment, communication, and organizational skills.


* Ability to manage multiple priorities in a fast-paced healthcare environment.


* Knowledge of Medicare, Medi-Cal, managed care, and discharge planning processes is a plus.
