Our therapy department works closely with nursing, physicians, and case management to develop individualized treatment plans that help residents achieve their highest level of function and independence.

Responsibilities


* Provide skilled occupational therapy treatments under the direction of a licensed Occupational Therapist


* Implement individualized treatment plans and physician orders


* Assist patients in improving activities of daily living (ADLs), self-care skills, upper extremity function, balance, coordination, and overall independence


* Monitor patient progress and communicate updates to the Occupational Therapist and interdisciplinary care team


* Complete accurate and timely daily treatment documentation and progress notes


* Participate in care conferences, discharge planning, and interdisciplinary team meetings


* Educate patients, families, and caregivers regarding therapy programs and home exercise activities


* Maintain therapy equipment and treatment areas according to facility standards


* Support facility goals for patient outcomes, customer service, and quality care

Qualifications


* Current California Certified Occupational Therapy Assistant (COTA) license in good standing


* Graduate of an accredited Occupational Therapy Assistant program


* Skilled nursing, post-acute, rehabilitation, or geriatric experience preferred
