Location: San Jacinto Valley Post Acute - San Jacinto, CA
Schedule: Per Diem / Part-Time
Compensation: $55.00 - $57.00 per hour (based on experience)

San Jacinto Valley Post Acute is seeking an experienced Physical Therapist (PT) to join our therapy team.

This role is ideal for a clinician passionate about delivering high-quality rehabilitation services in a skilled nursing setting.

Key Responsibilities


* Evaluate residents and develop individualized treatment plans


* Provide hands-on therapy services to improve mobility, strength, and function


* Supervise PTA staff and ensure quality of care


* Maintain accurate and timely documentation


* Collaborate with interdisciplinary teams to support resident outcomes

Qualifications


* Current California Physical Therapist license


* Experience in a skilled nursing or post-acute setting preferred


* Strong clinical and documentation skills


* Ability to lead and collaborate effectively
