Compensation:


* Full-Time: $70.00 - $75.00 per hour + Full Benefits


* Travel/PRN: Up to $95.00 per hour

Essential Responsibilities


* Evaluate patients within required regulatory timeframes following referral


* Develop individualized treatment plans based on patient needs and goals


* Deliver evidence-based physical therapy interventions in accordance with physician orders


* Monitor patient progress and adjust treatment plans as appropriate


* Supervise Physical Therapist Assistants (PTAs) and support staff


* Collaborate with physicians, nursing staff, therapy team members, and families


* Participate in care conferences, discharge planning, and interdisciplinary meetings


* Educate patients, families, and caregivers on therapeutic programs and safety measures


* Maintain accurate and timely documentation in compliance with state, federal, and facility requirements


* Assist with recommendations and procurement of adaptive and durable medical equipment


* Ensure therapy equipment is properly maintained and functioning safely

Qualifications


* Active California Physical Therapist License in good standing


* Bachelor's Degree in Physical Therapy required


* Master's or Doctorate in Physical Therapy preferred


* Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, or Long-Term Care experience preferred
