Position Summary

As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and overall function.

You will work closely with an interdisciplinary team to deliver personalized, patient-centered care.
Key Responsibilities


* Perform patient evaluations and develop individualized treatment plans


* Provide therapy to improve mobility, strength, balance, and functional independence


* Monitor patient progress and adjust treatment plans as needed


* Maintain accurate and timely clinical documentation


* Communicate effectively with physicians, residents, families, and care team members


* Participate in care planning and discharge planning


* Conduct staff education and in-service training


* Supervise PTAs, aides, and students as needed

Qualifications


* Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred)


* Active, unencumbered Physical Therapist license in South Carolina (required)


* Experience in a skilled nursing or rehab setting preferred


* Familiarity with PointClickCare (PCC) and Casamba preferred


* Strong clinical, communication, and leadership skills


* Commitment to patient-centered care
