Francis Healthcare is seeking a knowledgeable, experienced, and detail-oriented MDS Nurse (LVN/LPN) to join our dedicated clinical team.

Position Overview
The MDS Nurse (LVN/LPN) is responsible for coordinating and completing Minimum Data Set (MDS) assessments in accordance with federal and state regulations.
This role supports reimbursement processes, interdisciplinary care planning, regulatory compliance, and positive resident outcomes through accurate clinical documentation and communication.
Key Responsibilities
Clinical Assessment & Documentation
* Complete MDS assessments and Care Area Assessments (CAAs) within required timeframes.
* Ensure accurate and timely resident documentation to support care planning and reimbursement.
* Maintain compliance with CMS, Medicare, Medicaid, state, and facility guidelines.
* Monitor resident progress and update care plans as appropriate.
* Review clinical records for accuracy, completeness, and regulatory compliance.
* Coordinate assessment schedules and ensure timely submission of all required documentation.
Care Planning & Collaboration
* Collaborate with interdisciplinary teams to develop individualized resident care plans.
* Participate in resident care conferences and quality improvement initiatives.
* Work closely with nursing, therapy, dietary, social services, and other departments to gather and review resident information.
* Communicate effectively with residents and families regarding care planning and services.
* Support nursing staff with documentation standards and assessment processes.
Regulatory Compliance & Reimbursement
* Ensure MDS submissions meet all federal, state, regulatory, and reimbursement requirements.
* Assist during audits, surveys, and regulatory reviews by providing accurate documentation and reports.
* Stay current on changes related to MDS 3.0, RAI processes, PDPM, and reimbursement regulations.
* Educate and support staff on proper documentation standards and MDS requirements.
* Monitor reimbursement opportunities and collaborate with facility leadership to optimize reimbursement accuracy.
Qualifications
Required
* Current and active LVN/LPN license in the state of practice.
* Previous MDS experience in a Skilled Nursing Facility (SNF) or long-term care setting.
