Position Overview

Vasona Creek Healthcare is seeking a dedicated and compassionate Case Manager RN to join our full-time care team.

This role is ideal for a licensed vocational nurse who excels in care coordination, interdisciplinary collaboration, and patient advocacy within a skilled nursing or healthcare setting.

Key Responsibilities


* Coordinate and manage patient care plans from admission through discharge


* Collaborate with physicians, RNs, therapists, social services, and interdisciplinary teams


* Monitor patient progress, treatments, and outcomes to ensure quality care


* Communicate effectively with patients, families, and healthcare providers


* Ensure compliance with facility policies, regulatory requirements, and documentation standards


* Assist with utilization review and discharge planning as needed

Qualifications


* Current RN license in good standing


* Prior case management, skilled nursing, or long-term care experience preferred


* Strong organizational, communication, and documentation skills


* Ability to work collaboratively in a fast-paced healthcare environment


* Proficiency with electronic medical records (EMR) systems preferred
