We are seeking a dedicated Physical Therapist (PT) to join our full-time rehab team.

Position Summary

As a Physical Therapist (PT), you will evaluate, develop, and implement individualized treatment plans to help residents improve mobility, reduce pain, and achieve their highest level of independence.

Key Responsibilities


* Conduct patient evaluations and develop customized treatment plans


* Provide therapeutic interventions to improve strength, mobility, and function


* Monitor patient progress and adjust treatment plans as needed


* Educate residents and families on therapy techniques and goals


* Maintain accurate and timely clinical documentation


* Collaborate with interdisciplinary teams to ensure optimal patient outcomes

Qualifications


* Bachelor's degree required; advanced degree preferred


* Active, unencumbered Physical Therapist license in South Carolina


* Experience in a long-term care or rehab setting preferred


* Familiarity with PointClickCare and Casamba preferred


* Strong communication and clinical decision-making skills


* Passion for patient-centered care
