We are seeking a motivated Physical Therapist (PT) to join our team on a PRN basis.

Position Summary

As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, function, and overall quality of life.

You will play a key role in helping patients regain independence and achieve personalized therapy goals.

Key Responsibilities


* Perform patient evaluations and develop individualized treatment plans


* Deliver therapeutic interventions to improve strength, balance, and mobility


* Monitor patient progress and adjust treatment plans as needed


* Maintain accurate and timely documentation


* Collaborate with interdisciplinary teams to support patient outcomes


* Educate residents and families on therapy techniques and recovery goals

Qualifications


* Bachelor's degree required; advanced degree preferred


* Active, unencumbered Physical Therapist license in South Carolina


* Experience in a long-term care or rehab setting preferred


* Familiarity with PointClickCare (PCC) and Casamba preferred


* Strong communication and clinical skills


* Ability to work independently and as part of a team
