* Position: Physical Therapist (PT) - PRN


* Location: Concord, CA


* Compensation: Starting at $64-$65/hour


* Schedule: PRN / Per-Diem

Key Responsibilities


* Perform comprehensive physical therapy evaluations in a timely manner following physician referral


* Develop and implement individualized treatment plans focused on mobility, balance, strength, pain management, and functional independence


* Supervise and collaborate with Physical Therapist Assistants (PTAs) to ensure quality patient care and compliance with state practice standards


* Participate in interdisciplinary care planning, patient conferences, and utilization review meetings


* Recommend appropriate Durable Medical Equipment (DME) to support safe mobility and discharge planning


* Educate residents, families, and caregivers on exercise programs, safety strategies, and long-term maintenance plans


* Maintain accurate and timely documentation in accordance with regulatory and payer guidelines

Qualifications


* Current California Physical Therapist (PT) license in good standing


* Strong communication, organizational, and clinical decision-making skills


* Ability to manage caseloads effectively in a fast-paced environment


* Compassionate, patient-centered approach to rehabilitation and care
