Sonoma Post Acute is looking for a compassionate, dependable, and patient-focused individual to join our team as a Patient Appointment Aide!

In this important role, you will accompany residents to medical appointments and help ensure they feel safe, comfortable, and supported throughout the process.

Responsibilities:


* Accompany residents to outside medical appointments


* Provide companionship and support during transport and appointments


* Communicate professionally with healthcare providers and facility staff


* Assist with appointment paperwork and basic coordination


* Help ensure residents arrive safely and on time


* Maintain resident dignity, comfort, and confidentiality at all times

Qualifications:


* No certification or licensure required


* Compassionate and patient demeanor


* Reliable transportation record and punctuality


* Strong communication and interpersonal skills


* Ability to remain calm, organized, and professional


* Experience working with seniors or in healthcare is a plus, but not required
