White Blossom Care Center is looking for a compassionate and detail-oriented Social Services Assistant to join our dedicated team.

Position Summary

In this role, you will support the Social Services Director in delivering high-quality, resident-centered care.

You will play a key role in addressing residents' psychosocial needs, coordinating services, and ensuring compliance with all regulatory requirements.

Key Responsibilities


* Assist the Social Services Director with care planning and service delivery


* Support residents and families with transitions, emotional needs, and care-related concerns


* Complete documentation, assessments, and progress notes in compliance with state and federal regulations


* Collaborate with interdisciplinary team members to ensure comprehensive care


* Advocate for resident rights and promote overall well-being


* Participate in discharge planning and connect residents with community resources

Qualifications


* Required: Skilled Nursing Facility (SNF) experience


* Knowledge of state and federal long-term care regulations


* Strong communication, organizational, and documentation skills


* Compassionate, team-oriented, and resident-focused approach


* High school diploma required; related certification or coursework preferred
