Midtown Oaks Post Acute is seeking a compassionate and motivated Full-Time Occupational Therapist (OT) to join our skilled nursing and rehabilitation team in Sacramento, CA.

Occupational Therapist (OT) Responsibilities


* Evaluate residents and create individualized occupational therapy treatment plans


* Provide skilled OT treatments to improve mobility, function, and independence


* Collaborate with interdisciplinary teams including nursing, therapy, and physicians


* Educate patients and families on treatment plans and recovery strategies


* Maintain accurate documentation in compliance with state and federal regulations


* Monitor patient progress and adjust treatment plans as needed

Occupational Therapist (OT) Qualifications


* Active California Occupational Therapist (OT) license


* Strong clinical and patient care skills


* Excellent communication and teamwork abilities


* Skilled nursing facility (SNF) or post-acute experience preferred, but new grads are welcome to apply


* Passion for helping geriatric and rehabilitation patients achieve success
