Job Summary
As an Assistant Director of Health Services in an Assisted Living Facility, Resident Care Facility, or Memory Care Facility, you will lead and manage the health and well-being of residents, ensuring compliance, coordinating care, and providing leadership and training to the health services team
As the Assistant Director of Health Services, you will play a key role in providing leadership and mentorship to care staff, ensuring that the care we provide aligns with state regulations and organizational policies.
The ideal candidate will possess excellent clinical skills, strong leadership abilities, and a passion for working with seniors in a long-term care setting.
Essential Duties and Job Responsibilities
* Resident Assessments: Conduct thorough assessments of new and existing residents to identify healthcare needs, preferences, and goals.
Continuously update care plans based on changes in health status.
* Care Planning & Documentation: Develop, implement, and evaluate individualized, person-centered care plans in collaboration with residents, families, and interdisciplinary team members.
Maintain accurate and up-to-date nursing documentation in compliance with state regulations and facility policies.
* Regulatory Compliance: Ensure the facility adheres to all state and federal regulations, healthcare standards, and best practices.
Participate in audits and quality assurance reviews to maintain high levels of compliance.
* Medication Management: Oversee the Health Service Department, completing audits of medication administration for compliance with state and federal regulations.
Assist staff with medication management processes, ensuring the safe and accurate delivery of prescribed treatments and medications.
* Staff Leadership & Education: Provide direction, mentorship, and support to medication and care staff.
Assist with training on nursing protocols, medication administration, and emergency procedures.
Promote a culture of teamwork, accountability and communication.
* Collaboration & Communication: Foster open communication with residents, families, staff, and outside providers to ensure coordinated, collaborative care for residents in our community.
Participate in interdisciplinary team (IDT) meetings and family care conferences to discuss residents' progress and concerns.
* Healthcare Coordination: Coordinate with outside providers, including physicians, therapists, and hospitals, to ensure continuity of care and address any special healthcare needs.
