Position Overview


* Status: Full-Time, Benefitted


* Rate: $65 (DOE)


* Experience: Experienced PTs or New Grads Welcome!


* Location: San Jose, CA


* Role: Evaluate and treat residents, lead clinical training, and manage individualized rehabilitation plans.

Essential Duties & Responsibilities


* Clinical Excellence: Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders.


* Treatment Planning: Develop and implement individualized plans focused on improving mobility, strength, balance, and functional independence.


* Supervision & Training: Supervise Physical Therapy Assistants (PTAs) and assist the nursing department with the training of Restorative Aides.


* Collaboration: Communicate regularly with physicians, nursing staff, and families regarding progress, goals, and discharge planning.


* Documentation: Complete timely and accurate clinical documentation in compliance with facility, state, and federal requirements.


* Equipment Management: Recommend and facilitate the ordering of durable medical equipment (DME) such as walkers, canes, and splints to enhance resident independence.


* Education: Provide in-service training for facility staff and educate families on fall prevention and home exercise programs.

Qualifications


* Education: Graduate of an accredited Physical Therapy program.


* Licensure: Current and active California Physical Therapist license (or eligibility for new grads).


* Skills: Ability to relate positively and effectively with residents, families, and professional colleagues.
