Citrus Heights Post Acute is seeking a compassionate and driven Speech-Language Pathologist (SLP) to join our skilled nursing team.

In this role, you'll help patients improve communication, swallowing, and cognitive function while working alongside a collaborative interdisciplinary team.

Key Responsibilities


* Evaluate patients within 24 hours (or 1 business day) of physician referral


* Develop and implement individualized treatment plans


* Provide speech therapy services in accordance with physician orders


* Monitor patient progress and adjust treatment plans as needed


* Maintain accurate, timely documentation in EMR systems


* Collaborate with physicians, nursing staff, and rehab team members


* Participate in care conferences, rehab meetings, and discharge planning


* Educate patients, families, and caregivers on home programs and ongoing care


* Provide staff training and in-services as needed

Qualifications


* Master's or Doctorate degree in Speech-Language Pathology


* Active California SLP license (required)


* Strong clinical, communication, and documentation skills


* Skilled nursing or post-acute experience preferred (not required)


* Ability to thrive in a fast-paced, team-oriented environment

Certifications & Requirements


* Current SLP license in good standing


* Ongoing continuing education per state requirements


* Experience with or willingness to learn EMR systems
