Home Health Liaison

Work Schedule:

100% FTE, Monday-Friday 8:00am-5:00pm, every third Saturday 8 am - 5 pm.

You will work at UW Health Charles St.

Clinic 2550 Charles St.

Additional components of compensation may include:

Evening, night, and weekend shift differential

Overtime

Qualifications



* Graduate of an accredited School of Nursing/College.

Required


* BSN.

Preferred

Work Experience



* At least one year professional nurse experience within the last five years in an acute care setting.

Required


* One year of experience in home care and hospital discharge planning.

Preferred

Licenses & Certifications



* Valid RN license.

Required


* Valid State driver's license.
