Pay Range: $40.00-$42.00 per hour, DOE

Trellis is a beautiful skilled nursing facility located in Chino, California.

We are seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team.

This position supports quality patient care by implementing individualized treatment plans under the supervision of a licensed Occupational Therapist.

Position Purpose:

To assist in delivering high-quality, patient-centered occupational therapy services in accordance with established treatment plans, facility policies, and regulatory standards.

Duties and Responsibilities:


* Implement and carry out patient treatment plans under the supervision of a licensed Occupational Therapist


* Communicate effectively and professionally with patients, families, physicians, and interdisciplinary team members


* Provide timely, accurate written documentation in accordance with facility and departmental requirements


* Report any changes in patient status or treatment needs to the supervising therapist promptly


* Coordinate treatment goals with other members of the care team


* Accurately prepare and submit patient charges to support appropriate billing


* Support departmental operations, initiatives, and quality improvement efforts


* Continue developing clinical and professional skills through ongoing education and training

Requirements and Qualifications:


* Associate degree or higher in Occupational Therapy from an AOTA or ACOTE-accredited program


* Current certification and good standing with all applicable regulatory agencies and licensing boards


* Knowledge of resident rights and commitment to patient confidentiality


* Ability to read, write, speak, and understand English effectively


* Strong interpersonal skills and ability to maintain professional working relationships


* Demonstrated integrity and professionalism; adherence to company Code of Conduct and compliance policies


* Completion of all mandatory compliance and ethics training
