Position Details


* Role: Occupational Therapist (OTR)


* Location: White Blossom Care Center - San Jose, CA


* Compensation: Up to $65.00/hour (DOE)


* Schedule: Full-Time, Part-Time, or PRN


* Program Type: Stable, In-House Therapy

Collaborative Team Culture

Join a large, multidisciplinary team of experienced clinicians and enthusiastic new grads in a fun, community-oriented setting featuring multiple open-air patios.

Key Responsibilities


* Evaluation & Treatment: Perform comprehensive evaluations and deliver evidence-based occupational therapy interventions per physician orders


* Clinical Leadership: Supervise and support Certified Occupational Therapy Assistants (COTAs) in compliance with California State Practice Acts


* Interdisciplinary Collaboration: Participate in care conferences, utilization reviews, and rehab meetings to ensure coordinated, patient-centered care


* Documentation Excellence: Complete timely and accurate evaluations, daily notes, progress notes, and recertifications in accordance with OT Board standards


* DME & Patient Independence: Recommend, order, and train patients on durable medical equipment (DME) to promote safety and functional independence


* Education & Discharge Planning: Provide caregiver training and education to families and nursing staff to support positive discharge outcomes
