Wolf Creek Care Center is seeking a dedicated Physical Therapy Assistant (PTA) to join our dynamic therapy team.

We are committed to providing high‑quality, resident‑centered care in a supportive and collaborative skilled nursing environment.

Full‑time, part‑time, and PRN schedules are available.

Position Summary

As a PTA, you will work hands-on with patients to implement physical therapy plans designed by licensed Physical Therapists.

You'll play a key role in helping patients recover from illness, surgery, or injury, promoting mobility, function, and quality of life.

Responsibilities


* Provide direct patient care under the supervision of a licensed Physical Therapist


* Document patient progress and communicate updates to the therapy team


* Collaborate with nursing, therapy, and medical staff to ensure coordinated care


* Ensure compliance with safety, infection control, and facility policies


* Maintain compliance with state and federal regulations and professional standards.

Qualifications


* Associate degree from an accredited Physical Therapist Assistant program


* Active California PTA license


* Strong communication, teamwork, and documentation skills.


* Prior SNF experience preferred but new grads are welcome to apply!
