Job Summary

Cypress Ridge Care Center is seeking a skilled and compassionate Physical Therapist to join our therapy team on a PRN or Part-Time basis.

The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide high-quality therapeutic care to support mobility, function, and overall quality of life.

Responsibilities


* Evaluate residents and develop individualized physical therapy treatment plans


* Provide direct physical therapy services in accordance with physician orders


* Document evaluations, progress notes, and discharge summaries accurately and timely


* Collaborate with interdisciplinary team members to ensure coordinated resident care


* Educate residents and caregivers on treatment plans, exercises, and safety techniques


* Maintain compliance with facility policies, state regulations, and professional standards

Qualifications


* Current Physical Therapist license in good standing


* Graduate of an accredited Physical Therapy program


* Experience in a skilled nursing or long-term care setting preferred


* Strong communication and documentation skills


* Ability to work independently and as part of a team
