Napa Post Acute is Hiring an LVN MDS Coordinator!

Pay: $38-$45/hour
Schedule: Full-Time

Napa Post Acute is looking for a knowledgeable and detail-oriented LVN MDS Coordinator to join our dedicated team.

What You'll Do:


* Coordinate and complete MDS assessments in accordance with federal and state regulations


* Ensure accurate and timely submissions


* Collaborate with interdisciplinary team members to support care planning


* Monitor documentation to ensure compliance and optimize reimbursement


* Support clinical staff with MDS-related processes and education

What We're Looking For:


* Current California LVN license


* MDS experience preferred (or strong clinical background with willingness to learn)


* Detail-oriented with strong organizational skills


* Ability to work independently while being a team player


* Knowledge of PDPM and regulatory requirements is a plus
