LVN - On Call

Position Summary:
Concord Post Acute is seeking a dependable and skilled Licensed Vocational Nurse (LVN) to join our team on an on-call basis.

The LVN will provide direct nursing care to residents, support clinical staff, and ensure high-quality patient outcomes in a fast-paced post-acute care environment.

This role requires flexibility, strong clinical judgment, and a commitment to compassionate care.

Key Responsibilities:


* Provide direct patient care in accordance with established nursing standards and care plans


* Administer medications and treatments as prescribed by physicians


* Monitor and document patient conditions, reporting changes promptly to the RN or physician


* Assist with admissions, discharges, and transfers of residents


* Collaborate with interdisciplinary team members to ensure continuity of care


* Maintain accurate and timely documentation in compliance with regulatory requirements


* Ensure a safe, clean, and supportive environment for residents and staff


* Respond to call-offs and staffing needs on short notice

Qualifications:


* Current, active LVN license in the state of California


* Current CPR/BLS certification


* Minimum of 1 year experience in a skilled nursing or post-acute setting preferred


* Strong clinical and assessment skills


* Excellent communication and interpersonal abilities


* Ability to work independently and adapt to changing patient needs

Work Schedule:


* On-call, as needed


* Must be available to work weekends, holidays, and variable shifts as required
