What you’ll do


* Deliver person‑centred, psychologically informed support, helping customers build independence and progress towards their LIFE (Living Independently and Feeling Enabled) goal.


* Lead reviews and develop LIFE support plans, ensuring outcomes, risks and strengths are clearly identified and reviewed regularly


* Coordinate and guide Support Workers and volunteers, providing day‑to‑day advice, inductions and oversight to ensure high‑quality support delivery


* Take a lead role in multi‑agency working, including safeguarding, MARAC, MAPPA and other professional forums to promote customer safety and wellbeing


* Apply agreed screening, referral and allocation processes, making informed decisions about eligibility and support provision

You have


* Passion and experience in supporting others ideally in a supported housing setting


* Experience assessing referrals and coordinating support


* The ability to work under your own initiative, remain calm under pressure and have a resilient approach


* Strong communication and collaborative skills
