IT Clinical Informatics Analyst

Position: IT Clinical Informatics Analyst (HCHB SME)
We're seeking an experienced IT Clinical Informatics Analyst to serve as a Subject Matter Expert (SME) for Homecare Homebase (HCHB).

This role supports and optimizes the platform across Home Health and Hospice, improving workflows, system performance, and data integrity.
Key Responsibilities


* Act as SME for HCHB across Home Health and Hospice workflows


* Troubleshoot system issues and support configuration improvements


* Analyze and optimize clinical workflows for efficiency and standardization


* Collaborate with vendor on system updates and enhancements


* Support reporting, data validation, and accuracy


* Participate in system projects and informatics initiatives


* Ensure HIPAA compliance and maintain documentation

Qualifications


* Bachelor's degree or equivalent experience


* Strong hands-on HCHB experience required


* Experience in Home Health and/or Hospice


* Knowledge of OASIS, PDGM, and Hospice workflows


* Ability to optimize EHR systems beyond implementation


* Strong analytical and communication skills
