What You'll Do:


* Work with the interdisciplinary care team to ensure accurate patient assessment and care planning


* Maintain open and ongoing communication with patients and families


* Provide compassionate and appropriate nursing care


* Assist with medications and treatments


* Perform other patient care duties as assigned

What We're Looking For:


* Missouri Licensed Practical Nurse (LPN) license


* Caring, compassionate, and positive attitude


* Team player who works well in a collaborative environment


* Prior Matrix and SNF experience is a plus
