Position Overview

Vasona Creek Healthcare is seeking a compassionate and detail-oriented LVN Wound Care Nurse to join our clinical team.

The ideal candidate has a strong background in wound assessment and treatment, a commitment to quality resident care, and the ability to work collaboratively with the interdisciplinary team.

Key Responsibilities


* Perform comprehensive wound assessments and document findings accurately


* Implement and monitor individualized wound care treatment plans


* Clean, dress, and manage a variety of acute and chronic wounds


* Monitor healing progress and report changes to the physician and care team


* Educate residents, families, and staff on wound prevention and care


* Ensure compliance with infection control, safety standards, and facility policies


* Maintain accurate and timely clinical documentation

Qualifications


* Current California LVN license in good standing


* Wound care experience preferred (long-term care or skilled nursing experience a plus)


* Strong clinical assessment and documentation skills


* Excellent communication and teamwork abilities


* Ability to work independently and manage time effectively
