We are seeking a dedicated LVN Unit Manager to lead care coordination efforts and ensure our residents receive exceptional, personalized care.
What You'll Do


* Serve as a key point of contact for residents and families regarding care plans, therapies (PT/OT), and treatment updates


* Coordinate and oversee admissions, discharges, and referrals


* Participate in daily stand-up meetings to support team communication and continuity of care


* Monitor resident progress and adjust care plans as conditions change


* Perform utilization reviews to ensure timely and cost-effective care


* Ensure accurate, compliant, and timely documentation


* Communicate effectively with physicians, providers, and insurance/payor sources


* Support residents and families so they feel informed, safe, and confident in their care

What We're Looking For


* Active LVN/LPN license (RN a plus)


* Bachelor's Degree in Nursing


* Minimum 2 years of clinical experience


* Knowledge of Medicare, Medi-Cal, and Medicaid


* Experience with PointClickCare (PCC) preferred


* Strong communication, organization, and time management skills


* Ability to work independently in a fast-paced environment


* Team player with a collaborative mindset
