Santa Rosa Post Acute is looking for a detail-oriented and compassionate MDS Coordinator to join our team! If you're an LVN or RN (RN preferred) with a passion for quality care, strong clinical knowledge, and a knack for accuracy and organization, we'd love to meet you.

What You'll Do:


* Coordinate and complete MDS assessments in accordance with federal and state regulations


* Ensure accuracy of coding to support appropriate reimbursement


* Collaborate with interdisciplinary teams to develop and update care plans


* Monitor documentation and ensure compliance with regulatory standards


* Participate in care conferences and quality improvement initiatives

What We're Looking For:


* Active LVN or RN license in California (RN preferred)


* Experience with MDS 3.0 (preferred)


* Strong understanding of Medicare/Medicaid guidelines and reimbursement


* Excellent attention to detail and organizational skills


* Ability to work collaboratively with nursing and therapy teams
