Key Responsibilities


* Evaluate residents and create individualized treatment plans


* Provide therapy to improve strength, mobility, ADLs, and functional independence


* Collaborate with interdisciplinary team members on care plans and discharge goals


* Document treatments, progress, and outcomes accurately and timely


* Educate patients and families on home exercise programs and adaptive techniques

Qualifications


* Current Occupational Therapist (OT) license in good standing


* Strong communication and teamwork skills


* Skilled nursing or post-acute experience preferred (not required)


* Compassionate, patient-centered approach with a strong work ethic
