Case Manager

Position Summary

Mission Valley Post Acute is seeking a compassionate and detail-oriented Case Manager to join our team on a part-time basis.

The Case Manager plays a critical role in coordinating resident care, facilitating communication between interdisciplinary teams, and ensuring smooth transitions throughout the continuum of care.

Key Responsibilities


* Coordinate and manage resident care plans in collaboration with nursing, therapy, and social services teams


* Monitor resident progress and adjust care plans as needed


* Facilitate discharge planning, including coordination with families, physicians, and outside agencies


* Ensure compliance with all regulatory and documentation requirements


* Maintain accurate and timely case management records


* Act as a liaison between residents, families, and healthcare providers


* Assist with insurance verification and authorization processes when applicable


* Identify and address barriers to effective care and safe discharge


* Participate in care conferences and interdisciplinary meetings

Qualifications


* Previous experience in case management, healthcare coordination, or social services preferred


* Knowledge of post-acute care processes and regulations is a plus


* Strong organizational and multitasking skills


* Excellent communication and interpersonal abilities


* Ability to work independently and collaboratively within a team


* Proficiency in basic computer systems and documentation
