White Blossom Care Center is seeking a compassionate and experienced Social Services Assistant to join our dedicated care team.

Key Responsibilities


* Support the Social Services Director in planning and delivering resident-centered services


* Assist residents and families with care planning, transitions, and psychosocial needs


* Help complete required documentation, assessments, and progress notes in compliance with state and federal regulations


* Communicate with interdisciplinary team members to ensure resident needs are met


* Advocate for resident rights and support their emotional well‑being


* Participate in discharge planning and community resource coordination

Qualifications


* Skilled Nursing Facility experience required


* Knowledge of state & federal regulations related to long‑term care social services


* Strong communication, documentation, and organizational skills


* Compassionate, resident-focused, and able to work collaboratively within a care team


* High school diploma required; related coursework or certification preferred
