Position Summary:
NeighborHealth is seeking an OB Care Navigator to support pregnant patients by identifying health-related social needs (HRSN), addressing care coordination gaps, and connecting patients to clinical, behavioral health, and community-based services.

What You'll Do

The OB Care Navigator works closely with OB providers, multidisciplinary care teams, and community partners to ensure patients receive timely, culturally responsive, and coordinated care throughout pregnancy.

The OB Care Navigator supports pregnant patients throughout pregnancy by addressing social, behavioral health, and care coordination needs to reduce barriers to care and improve maternal health outcomes.
