RN Clinical Nurse - Advanced Heart Failure & Palliative Care

RN – Advanced Heart Failure & Palliative Care (Day Shift) 

This RN role supports an outpatient Advanced Heart Failure & Palliative Care clinic focused on improving outcomes, quality of life, and patient understanding of heart failure, LVAD care, and palliative needs.

The nurse provides direct clinical care, patient education, and care coordination in a Monday–Friday setting with no weekend or major holiday hours.

Key Responsibilities
Provide direct patient care including assessments, vital signs, phlebotomy, medication administration, EKGs, and 6‑minute walk tests
• Deliver patient education on disease management and treatment plans
• Conduct telephone and electronic triage through EPIC
• Coordinate care with social workers, nurse navigators, and other team members
• Supervise and delegate tasks to non‑licensed staff
• Use the full nursing process to deliver individualized, evidence‑based care
• Maintain accurate documentation and follow current clinical standards
• Participate in interdisciplinary communication, rounds, and care planning
• Support performance improvement, shared governance, and evidence‑based practice
• Assist in developing protocols to improve outcomes and reduce readmissions
• Use resources efficiently and promote a safe, high‑quality care environment
 

Qualifications
• Bachelor’s degree in Nursing or Nursing Diploma
• Active RN license (single‑state or compact)
• BLS certification
• Direct patient care experience preferred
• Strong communication, critical thinking, time management, and computer skills
• Ability to work in a fast‑paced environment and respond effectively to emergencies
