Health & Wellness Coordinator (HWC) - Brookdale Lakeridge

The Health & Wellness Coordinator is responsible for supporting the overall health and well-being of residents while ensuring quality care services are delivered in a safe, compassionate, and efficient manner.

This role requires close collaboration with the Resident Care Coordinator (RCC) and Health & Wellness Director (HWD) to maintain continuity of care and respond to staffing or resident needs.

Key Responsibilities:


* Assist with daily health and wellness operations, including working the medication/treatment cart as needed


* Partner with RCC and HWD to manage staffing needs, including coverage for call-outs


* Provide support to residents, ensuring individualized care and service excellence


* Maintain flexibility to adapt to changing schedules and community needs

Schedule & Requirements:


* Primary shift: 10:00 AM - 6:00 PM, with potential for swing shift as needed


* Must be flexible with scheduling; current early shift model (7:00 AM - 3:00 PM) is not standard for this role


* Required to work every other weekend (10:00 AM - 6:00 PM)


* No formal on-call schedule; however, flexibility is expected for call-outs if needed


* Potential for an on-call rotation every three weeks to support emergencies and staffing needs
