Social Worker

Job Title: Licensed Social Worker (LSW/LMSW) - Post Acute Care
Facility: Orchard View Post Acute
Pay: $35-$45/hour (depending on experience)
Schedule: Full-time

Orchard View Post Acute is seeking a compassionate, licensed Social Worker to support residents and families through every stage of post-acute care.

This role is essential in helping ensure safe discharges, coordinating community resources, and providing emotional support in a fast-paced skilled nursing environment.

Key Responsibilities


* Complete psychosocial assessments and develop care plans in collaboration with the interdisciplinary team


* Provide counseling, crisis intervention, and emotional support to residents and families


* Coordinate discharge planning, including placement, resources, and follow-up needs


* Assist with resident advocacy, care conferences, and family communication


* Support compliance with federal/state regulations and facility policies


* Maintain accurate, timely documentation in accordance with standards and guidelines

Qualifications


* Active Social Work license in the state of Idaho


* Experience in skilled nursing, post acute, or long-term care preferred


* Strong knowledge of discharge planning and community resources


* Excellent communication, organization, and documentation skills


* Ability to work collaboratively with clinical and administrative teams
