Citrus Heights Post Acute is looking for a dedicated and compassionate Speech-Language Pathologist (SLP) to join our team! You'll play a vital role in helping patients regain communication, swallowing, and cognitive function in a collaborative, supportive skilled nursing environment.

What You'll Do


* Evaluate patients within 24 hours (or 1 business day) of physician referral


* Develop and implement individualized treatment plans


* Provide therapy per physician orders


* Monitor, document, and adjust patient progress as needed


* Collaborate with physicians, nursing staff, and interdisciplinary team members


* Participate in care conferences, rehab meetings, and discharge planning


* Educate patients, families, and caregivers for continued care


* Provide in-services and training for facility staff


* Maintain accurate documentation using EMR systems

Qualifications


* Master's or Doctorate degree in Speech-Language Pathology


* Active California SLP license (required)


* Strong clinical, communication, and documentation skills


* Skilled nursing or post-acute experience preferred, but not required


* Team-oriented with the ability to thrive in a fast-paced environment

Certifications & Requirements


* Current SLP license in good standing with the state


* Ongoing continuing education per state requirements


* Familiarity with EMR systems
