The Opportunity:



* Position: Physical Therapist (PT)


* Location: Concord, CA (Contra Costa County)


* Compensation: Starting at $60.00 per hour (DOE)


* Availability: Full-Time, Part-Time, and PRN


* Shifts: Flexible 5-day schedules (Tue-Sat or Sun-Thu) — Skip the Monday rush!

Your Impact & Responsibilities:



* Clinical Leadership: Perform comprehensive evaluations and develop physician-approved treatment plans tailored to each resident's recovery journey.


* Team Supervision: Provide direct patient care and expert supervision of Physical Therapy Assistants (PTAs) in accordance with California State Practice Acts.


* Interdisciplinary Teamwork: Collaborate with the medical and nursing teams to communicate patient progress, goals, and discharge planning.


* Resident Advocacy: Lead discharge planning and recommend necessary durable medical equipment (DME) to ensure a safe transition home.


* Education: Provide vital education to nursing staff, patients, and families regarding long-term maintenance and caregiver safety.


* Documentation Excellence: Maintain compliant, precise, and timely documentation of all clinical services.
