Your Impact & Responsibilities:



* Comprehensive Evaluations: Complete initial assessments and develop individualized treatment plans for residents with functional, cognitive, or ADL limitations.


* Therapeutic Intervention: Provide skilled treatments targeting ADLs, functional mobility, strength, cognition, and safety.


* Equipment & Innovation: Recommend adaptive equipment and environmental modifications to ensure a safe transition back to home life.


* Interdisciplinary Collaboration: Work closely with PT, SLP, Nursing, and Medical teams to deliver a holistic, coordinated approach to recovery.


* Expert Documentation: Maintain precise, timely documentation of resident progress in accordance with California state standards.
