Your Impact & Responsibilities:



* Nutritional Assessments: Conduct comprehensive nutritional assessments for new admissions and high-risk residents, identifying specific caloric and protein needs.


* MNT Implementation: Develop and monitor individualized medical nutrition therapy plans for residents with complex conditions (e.g., diabetes, renal disease, wound healing, and enteral nutrition).


* MDS & Documentation: Complete MDS (Minimum Data Set) sections, CAAs, and care plans accurately and timely in accordance with state and federal regulations.


* Weight Management: Monitor resident weights and laboratory data, providing immediate intervention and clinical recommendations for any significant changes.


* Culinary Collaboration: Partner with the Dietary Manager to ensure menu compliance, palatability, and adherence to therapeutic diets.


* Interdisciplinary Team (IDT): Participate in care conferences and clinical meetings to advocate for the nutritional well-being of our residents.

Qualifications:



* Certification: Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration (CDR).


* Licensure: Current California state requirements for dietetic practice.


* Clinical Expertise: Experience in a skilled nursing, sub-acute, or hospital setting is preferred.


* Communication: Strong interpersonal skills with the ability to educate residents, families, and staff on nutritional goals.
