The Social Worker will support residents and their families by addressing psychosocial needs, coordinating care plans, and enhancing quality of life.

Candidates with an LPN or RN license and a degree in Social Work or related field are highly encouraged to apply, as this role bridges medical care and emotional support.

* Conduct psychosocial assessments and develop individualized care plans


* Provide counseling, emotional support, and crisis intervention


* Collaborate with nursing staff, physicians, and interdisciplinary teams

* Bachelor's degree in Social Work, Psychology, or related field (required)


* Active LPN or RN license (preferred)
