What You'll Be Doing:


* Lead and coordinate the MDS assessment process from start to finish


* Ensure accuracy, timeliness, and compliance with all state and federal guidelines


* Collaborate with nursing, therapy, and administrative teams to develop comprehensive care plans


* Monitor case mix, quality measures, and reimbursement accuracy


* Serve as the go-to expert on MDS-related processes and best practices

What You Bring to the Table:


* Active RN or LPN license in good standing


* MDS experience in a long-term care setting preferred


* Strong organizational and analytical skills


* Confidence working independently and as part of an interdisciplinary team


* Solid understanding of CMS regulations and PDPM
