Colonial Vista Post Acute are seeking a skilled and compassionate Occupational Therapist (OT) to join our rehab teams and help residents regain independence and confidence in daily living.

Compensation
• $45-$55 per hour, based on experience

What You'll Do

• Evaluate residents and develop individualized occupational therapy treatment plans
• Provide skilled OT services focused on ADLs, functional mobility, and safety
• Modify treatment plans based on resident progress and clinical judgment
• Document evaluations, treatments, and progress notes accurately and timely
• Collaborate with interdisciplinary teams to support resident-centered goals
• Educate residents, families, and caregivers as appropriate

Requirements

• Current Occupational Therapist (OT) license
• Skilled nursing or post-acute experience preferred
• Strong clinical, communication, and documentation skills
• Commitment to quality care and positive resident outcomes
