Are you ready to make a meaningful difference in the lives of our senior community? Las Colinas Post-Acute, a vibrant 216-bed skilled nursing facility in Ontario, CA, is seeking passionate LVNs/LPTs who are ready to bring their skills, dedication, and heart for care to our team.

Pay Range:
$29-$33 per hour, DOE

What You'll Be Doing:


* Welcoming new graduates and supporting their transition into senior care


* Communicating clearly and professionally with residents, families, and team members


* Administering medications and documenting care electronically


* Managing acute and chronic resident care needs


* Utilizing critical thinking skills to support quality outcomes


* Leading and supporting CNAs to ensure excellent resident care

What You'll Need:


* A current LVN or LPT license in good standing


* Strong clinical skills with a compassionate, resident-centered approach


* A positive attitude and ability to work effectively in a team environment
