Physical Therapist (PT) - PRN Weekends

Location: Sunnyvale Post Acute - Sunnyvale, CA
Pay Rate: $57-$67 per hour (DOE)
Schedule: PRN / Weekends

We are seeking a talented PRN Weekend Physical Therapist to join our rehab department.

Position Summary

The PRN Physical Therapist evaluates, plans, and provides patient‑centered physical therapy services for individuals requiring rehabilitation in a post‑acute/long‑term care setting.

You will collaborate closely with an interdisciplinary team to ensure the best rehabilitation outcomes for our residents.

Responsibilities


* Complete PT evaluations and develop individualized treatment plans


* Provide effective, evidence‑based therapeutic interventions


* Document treatments, progress notes, and evaluations in accordance with facility and regulatory requirements


* Communicate patient progress with interdisciplinary team members


* Educate patients, families, and caregivers on therapy goals, safety, and home programs


* Ensure safe, ethical, and high‑quality care at all times

Qualifications


* Current California Physical Therapist (PT) license


* Graduate of an accredited PT program (DPT or MSPT)


* Previous SNF or post‑acute rehab experience preferred (not required)


* Strong clinical reasoning and communication skills


* Ability to work independently and collaboratively
