The Opportunity:



* Position: Full-Time, Part-Time, or PRN


* Location: Lone Tree Post Acute — Antioch, CA


* Rate: $ 55.00 -$ 65.00 per hour (Starting rate DOE and position)

Your Impact & Responsibilities:



* Clinical Leadership: Evaluate patients and develop comprehensive treatment plans to optimize recovery.


* Therapeutic Excellence: Deliver physical therapy interventions consistent with physician orders and evidence-based practices.


* Mentorship: Supervise Physical Therapy Assistants (PTAs) and provide training for Restorative Aides to ensure continuity of care.


* Documentation: Maintain precise and timely evaluations, progress notes, and recertifications per state and payer requirements.


* Collaboration: Communicate patient progress with the interdisciplinary team and participate in care conferences and utilization reviews.


* Patient Advocacy: Educate families and nursing staff on maintenance programs and facilitate the ordering of necessary durable medical equipment (DME).

Qualifications:



* Licensure: Current, valid California Physical Therapist (PT) license (or eligibility to obtain).
