The Opportunity:



* Position: Full-Time (CNA)


* Location: Pleasant Hill Post Acute — Pleasant Hill, CA


* Rate: $ 22.00-$ 25.00 per hour (Commensurate with experience)


* The "4/2" Advantage: Enjoy a 4 Days On / 2 Days Off rotating schedule, ensuring consistent hours and predictable days off!


* Available Shifts:
+ Full Time - AM: 6:30 AM - 2:30 PM
+ Part Time/On Call - All Shifts

Your Impact & Responsibilities:



* Person-Centered Care: Provide routine nursing care tailored to each resident's unique assessment and care plan.


* Clinical Observation: Actively monitor residents and promptly report any changes in condition to the Nurse Supervisor.


* Documentation Excellence: Maintain accurate clinical records and flow sheets to ensure the highest standards of safety.


* Resident Advocacy: Ensure resident safety through proper identification and immediate reporting of any incidents.


* Hospitality & Admissions: Prepare rooms for new admissions, welcome new residents, and assist with transfers or discharges.


* Integrity: Maintain strict confidentiality (PHI) and perform nursing procedures with clinical excellence.

Qualifications:



* Licensure: Must possess a current, valid California Certified Nursing Assistant (CNA) Certification.
