Willow Creek Healthcare Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our nursing team on the AM shift.

Key Responsibilities


* Provide direct nursing care in accordance with resident care plans and physician orders


* Administer medications and treatments accurately and timely


* Monitor residents for changes in condition and promptly report findings


* Document care, observations, and changes in condition in the EMR system


* Assist with admissions, discharges, and transfers as needed


* Collaborate with CNAs and interdisciplinary team members to deliver resident-centered care


* Maintain a clean, safe, and supportive environment


* Follow all infection control, safety, and confidentiality protocols

Qualifications


* Active California LVN license in good standing


* Current CPR certification


* Skilled nursing or long-term care experience preferred


* Familiarity with EMR systems (PointClickCare preferred)


* Strong communication, organization, and time-management skills


* Compassionate, reliable, and team-oriented
