What you?ll do


* Lead support planning and coordinate Support Workers to deliver person?centred care


* Manage caseloads, balancing safeguarding, admin, meetings and team guidance


* Support customers with daily living skills, including budgeting, cooking and appointments


* Work with multi?agency partners to safeguard, empower and support move?on into the community

You have


* Experience supporting others, ideally within supported housing


* Ability to assess referrals and coordinate support


* Self?motivated, resilient and calm under pressure

The practical bits


* Rota-based role, Monday?Sunday, with alternate weekends


* Core hours with flexible shifts between 8am?10pm


* Based mainly in supported accommodation, with community visits across Hartlepool


* Enhanced DBS with barring list required (paid for)


* Full driving licence and business?insured vehicle required (mileage paid)
