🩺 Physical Therapist (PT) Responsibilities


* Evaluate and treat patients in a skilled nursing/post-acute environment


* Develop and implement individualized treatment plans


* Complete evaluations, progress notes, and discharge documentation accurately and on time


* Collaborate with nursing, therapy, and interdisciplinary teams


* Educate patients and families on therapy goals and home exercise programs

✅ Physical Therapist (PT) Requirements


* Active California Physical Therapist license (or pending)


* Skilled nursing or post-acute experience preferred, but not required


* Strong communication, documentation, and time-management skills


* Team-oriented, compassionate, and patient-centered approach
